School-Based Health Center Enrollment

Hello Macdonough Community!

Macdonough School has a school-based health center!  The SBHC provides medical services inside your child’s school during school hours.  Services include sick visits (for concerns such as sore throat, cold and flu symptoms, or rashes), physicals, immunizations, injury evaluation, and specialty consults and referrals.  We can also monitor chronic health concerns such as asthma. In order to receive services, your child does need to be enrolled.  You can complete the online enrollment form here:  https://www.sbhc1.com/. Paper enrollment forms are also available upon request (we can send them home with your child).  If you are not sure if your child is enrolled, please call the SBHC at 860-347-8553.
